Patient Forms

You can save time at your next visit by printing out and completing the patient forms in advance of your appointment.

Insurance

For those patients who have insurance and are allowed to select their dentist, we accept payment from most major carriers. However, we are not under contract with any insurance company; rather, we participate in the nonrestrictive portion of the plans. We strongly feel this is in the patients’ best interest.

Our office will gladly submit your insurance claim for you. Although we do our best to estimate the insurance portion, the payment amounts vary widely depending on which level or quality of plan your employer has selected.

Insurance companies have become increasingly unresponsive to our submissions. Because the patient is the insurance company’s customer, you are better able to get more accurate estimates and faster payment than our office can.

If your insurance company does not respond to your claim for payment within 30 days, please pay any outstanding balance owed for the services provided to you. Please contact your insurance company regarding the delay in payment. Any overpayment that may result from a late arriving insurance check can be refunded to you.

Payment Options

  • Cash/Check
  • 5% discount if total fee is paid in full by cash or check before the appointment
  • Visa, Mastercard, American Express, Discover
  • 6 or 12 month interest-free financing through Care Credit
  • 2 or 3 year low interest financing through Care Credit (Care Credit is a nationwide program for financing medical, dental, and eyecare expenses)

New Patients

We are happy to become your new dental home, and want your first experience with us to be both pleasant and productive. We reserve special times for new patients, and can usually appoint you within a week of calling the office.

For your first visit, you will be scheduled for 70-90 minutes with one of our hygienists. We will carefully review your medical history and determine what x-ray images are necessary to provide a complete view of your mouth’s condition. If you have current x-rays at another office, please bring them with you or have that office forward them to us ahead of time. We cannot do this for you due to Privacy Laws. The doctor will determine if the copies are of sufficient quality for today’s visit. If not, or if not available, we will need to make new original images for a proper exam and diagnosis of your oral condition.

If a routine cleaning is appropriate, this can be accomplished at this visit. Should other gum treatment be needed, this will be discussed with you first. After the hygienist completes her part, the doctor will do a complete exam of your teeth and surrounding tissues, including a screening for oral cancer. If your treatment needs are complex or involve multiple options, we will invite you back for a special consultation with the doctor at no additional charge. We feel it is important to discuss the pros and cons of any options you have, show you models, and answer all of your questions. We will also reserve time with one of our Treatment Coordinators to review insurance and financial options available.

Canceled / Broken Appointments

Nearly all our patients faithfully keep their dental appointments and we appreciate it. Keeping regular appointments is important in maintaining your oral health, and in being able to provide you the appointment times and dentist/hygienist that you prefer.

A specific amount of time is reserved especially for you and we strongly encourage all patients to keep their appointments. As we do not double book patients, a broken appointment means that the time is not utilized. We understand that life happens, so if rescheduling is necessary, please give 48 hours notice. Appointments canceled with less than 24 hours notice may be assessed a $50 cancellation fee. If missed appointments become habitual, you will be asked to pre-pay for your next appointment.

Emergencies

Same day appointments for an urgent emergency are reserved for current active patients. Please call first to make arrangements. After hours, please call the main number for information.
